Nursing Recommendation Letter: 6 Samples for Nurses

A nursing recommendation letter works when the writer describes one real shift or clinical day in which the nurse did the job well, then says plainly that they would hire or work with that person again. The constraint is that the reader is often a nurse manager or an admissions panel who has read hundreds of these, and "caring and hardworking" tells them nothing. O*NET, the US Department of Labor's occupation database, sums up the registered nurse role as "Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records", and the best letters show the person doing exactly those things. Below are six complete nursing recommendation letter samples, checked in October 2026. Written by Mustafa Tarabya, who hired for his own company, read every application himself and now writes CVBooster's guides.

Every nurse you have ever met is "compassionate". The letter that gets read twice says she noticed the patient in bed 4 was quieter than usual, checked his sugar, and was right.

Charge nurse recommending a staff RN

This is the most common request: a nurse moving to a new hospital or specialty asks a charge nurse or manager to write. Lead with the working relationship, then one clinical story, then the hire-again sentence.

To the Nurse Recruitment Team, Riverside Medical Center

I am pleased to recommend Daniel Okafor, RN, for the critical care nurse position. I have been the night charge nurse on 6 West, a 32-bed medical-surgical unit at St. Luke's Hospital, for seven years, and Daniel has worked on my shifts for the last three.

Daniel is the nurse I assign to the patient I am most worried about. Last winter he was caring for a post-operative patient whose blood pressure was technically within range but had been drifting down for two hours. He flagged it, rechecked, and called the rapid response team before the patient's condition dropped further. The patient was transferred to the ICU and recovered. The intensivist told me afterwards that the early call made the difference.

He is also the nurse new graduates ask for. He precepted four of them last year and every one is still on the unit.

I would hire Daniel again without hesitation. Please contact me at the number below.

Sincerely, Maria Chen, RN, BSN Night Charge Nurse, 6 West, St. Luke's Hospital (555) 318-2207

Clinical instructor recommending a new graduate

New graduates have no nursing job to point to, so the letter comes from a clinical instructor or a preceptor. The reader for a nurse residency program wants to know how the student behaves on a real unit.

Dear Nurse Residency Selection Committee,

I am writing to recommend Hannah Lee for your Nurse Residency Program. I was Hannah's clinical instructor for two rotations at Mercy General Hospital, adult medical-surgical and cardiac telemetry, and I supervised her across 18 clinical days.

Hannah's assessments were the most complete in her cohort. On a telemetry shift, she noticed a patient's new irregular rhythm before the monitor technician called, assessed him, and reported it to the primary nurse with a clear SBAR summary. She did not wait to be told what to do, and she did not overstep either.

She also asks the right questions. When she did not understand why a medication was being held, she asked the nurse and then looked up the parameters herself before the next dose.

Hannah graduates in May and expects to sit the NCLEX-RN in June. I recommend her for your program with full confidence.

Sincerely, Professor Angela Ruiz, MSN, RN Clinical Instructor, Lakeshore College School of Nursing aruiz@example.edu

Recommendation for a nursing program applicant

Applicants to an ADN or BSN program often have no clinical record yet. The letter comes from a teacher, a CNA supervisor or a volunteer coordinator, and it should speak to reliability, science ability and how the applicant treats people.

Dear Admissions Committee,

I am happy to recommend Jasmine Carter for admission to your Bachelor of Science in Nursing program. I am the Director of Nursing at Oak Grove Rehabilitation Center, and Jasmine has worked here as a certified nursing assistant for two years.

Jasmine is dependable in the ways that matter in long-term care. She has not missed a scheduled shift, she reports changes in residents promptly, and her documentation is accurate. When one of her residents began refusing meals, Jasmine noticed the pattern over three days, told the charge nurse, and the resident was assessed and treated for a dental infection.

She has told me for a year that she wants to become a registered nurse, and she has been taking her prerequisite science courses in the evenings while working full time.

I recommend her without reservation. Please contact me if you need more information.

Sincerely, Linda Brooks, RN Director of Nursing, Oak Grove Rehabilitation Center (555) 420-1186

Recommendation for a nursing student

Students need letters for scholarships, externships and summer programs. A professor or instructor writes it, and the strongest version names one piece of work.

Complete Entry level: professor for a nursing student externship example, 148 words, laid out as it would sit in the application form

Dear Externship Coordinator,

I recommend Tyler Nguyen for your summer nurse externship. Tyler is a third-year student in our BSN program and was in my pathophysiology course and my adult health clinical group this year.

Tyler earned one of the top grades in pathophysiology, but what I remember is his care plan for a patient with heart failure. He tied every intervention to the patient's own goal, which was to walk to his mailbox again, and he followed up with the patient on the next clinical day to see whether the teaching had worked.

On the unit, Tyler is calm, prepared and honest when he does not know something. Nurses on the unit asked whether he could be placed with them again.

He will make an excellent extern. I am happy to answer any questions.

Sincerely, Dr. Karen Wells, DNP, RN Associate Professor, Department of Nursing, Bayview University

Recommendation for a nurse practitioner program

NP programs want evidence of clinical judgment and leadership. The writer is usually a nurse manager, a physician or an NP who has worked with the applicant.

Complete Senior: nurse manager for an FNP program applicant example, 160 words, laid out as it would sit in the application form

Senior: nurse manager for an FNP program applicant (160 words)

Dear Graduate Admissions Committee,

I am writing to recommend Sofia Alvarez, RN, BSN, for your Family Nurse Practitioner program. I am the nurse manager of the emergency department at County General Hospital and have supervised Sofia for five years.

Sofia is one of three charge nurses I trust to run the department on our busiest nights. She triages well, she keeps the team calm, and she knows when to escalate. Two years ago she noticed that patients with uncontrolled diabetes were returning to our department every few weeks. She worked with our case manager to create a referral pathway to the hospital's primary care clinic, and return visits for that group dropped noticeably the following year.

That project is why I believe she will be an outstanding nurse practitioner: she already thinks about what happens to patients after they leave.

I recommend her with my highest endorsement.

Sincerely, Robert Kim, MSN, RN, CEN Nurse Manager, Emergency Department, County General Hospital

Short reference letter for a nurse

Some employers ask for a brief written reference instead of a full letter. Keep it to dates, role, one strength with proof, and the rehire statement.

Complete Mid level: short reference for a home health nurse example, 99 words, laid out as it would sit in the application form

Mid level: short reference for a home health nurse (99 words)

To Whom It May Concern,

Priya Shah, RN, worked as a home health nurse with Sunrise Home Care from March 2022 to August 2026, reporting to me.

Priya managed a caseload of patients with complex wound care and diabetes across a large rural area. Her visit notes were complete and on time, and our auditors used one of her wound care records as a training example.

She left on good terms to move closer to family. I would rehire her.

Please contact me if you have any questions.

Sincerely, James Porter, RN Clinical Manager, Sunrise Home Care (555) 702-9934

How to write a nursing recommendation letter

Use the same five parts every time, in this order:

Never include patient names or anything that could identify a patient. A good story only needs the clinical facts.

If you are the nurse asking for a letter, give your writer your resume, the job advert or program description, and two or three situations they might remember. Most writers say yes and then stare at a blank page; a short list of reminders helps. Our nursing resume guide shows how to put those same situations on your resume, so the letter and the resume tell one story.

Letter, reference or form

Many hospitals now collect references through an online form rather than a letter. If that happens, the same rules apply: one specific story per question, short sentences, and the rehire answer stated clearly. Nursing programs usually want a signed letter uploaded by the writer directly, because the applicant waives the right to see it.

For general letters outside nursing, our reference letter examples cover employers, teachers and landlords. For the difference between a reference and a recommendation, see the reference letter guide. Students applying to medical residency programs need a different structure, covered in the residency recommendation letter guide.

Nursing students should also know the licensing step that follows graduation: every new nurse must pass the NCLEX before practising, and residency program letters often mention when the candidate will sit it. The AACN, the national body for academic nursing, publishes program standards if you want to see what nursing schools look for in applicants.

Frequently asked questions

How do I write a recommendation letter for a nurse?

State how you know the nurse and for how long, then describe one real clinical situation where they assessed a patient and acted well, with details changed for privacy. Add one professional quality with an example, connect it to the job or program, and end by saying you would hire or work with them again.

What should a recommendation letter for nursing school include?

Include your relationship to the applicant, evidence of reliability and how they treat people, any science or academic strength you have seen, and why they will do well in a demanding program. Letters from CNA supervisors, science teachers and volunteer coordinators carry weight because they describe the applicant under real pressure.

Who should write a nursing recommendation letter?

For a nursing job, a charge nurse, nurse manager or preceptor who supervised you. For a new graduate role, a clinical instructor or preceptor. For nursing school, a teacher, professor or work supervisor. For an NP program, a nurse manager, physician or NP who has seen your clinical judgment directly.

How long should a nursing recommendation letter be?

One page, usually 150 to 300 words. Admissions panels and nurse managers read many letters, and a specific page beats a general two pages. Some employers ask for a short reference of about 100 words, which only needs dates, role, one proven strength and the rehire statement.

Can a nursing student ask a clinical instructor for a recommendation?

Yes, and a clinical instructor is often the best choice because they have seen you on a real unit. Ask early, at least a month before the deadline, give them your resume and the program details, and remind them of one or two clinical days where you did well.
